Correct positioning. The derail needs to be placed the right distance from the protected zone, on the correct rail, and in the correct orientation for the direction of travel being protected. Placing it on the wrong rail or in the wrong orientation means it will not engage correctly when a vehicle approaches.
Locking. As mentioned, the lock is not optional. An unlocked derail in a possession is a compliance failure and, more importantly, a protection gap. The person responsible for the possession protection holds the key. Only they can remove the lock and lift the derail from service.
Visual indication. The derail flag or sign must be correctly positioned and clearly visible from the approach direction. Workers and plant operators need to be able to see at a distance whether the derail is in service.

It is worth being clear about this: a derail is not a standalone protection solution. It is one element of a layered protection system, and it is always the last resort rather than the first line of defence.
